6.13 ATHLETIC CODE POLICY

 Registration — Athletes must register for athletics prior to an athlete’s participation in a practice or event. To register:

• A current physical form must be on file in the athletic office (valid for one year).

• An athletic information sheet must be completed and on file in the athletic office.

• A medical history form must be completed and on file in the athletic office.

• An athletic participation agreement must be completed and on file in the athletic office.

• A participation fee of $75 per sport must be paid to the athletic department.

• The student must show proof of accident insurance covered either by a policy purchased through the school-approved plan or a written statement by a parent/guardian that the student is covered under a family insurance plan.

Petitioning Out — Petitioning out of Physical Education may occur when:

• The athlete is a junior or senior.

• The athlete is enrolled in six classes per day plus P.E. (The student must be in class 300 minutes per day.)

• The athlete has received a passing grade in P.E. during the school year.

• The athlete has no unexcused absences in P.E. for the school year.

• The athlete has completed all the necessary forms, paid the $75 participation fee, and turned them into the Athletic               Director.

• The athlete fully participates as a member of an IHSA-sanctioned sport by attending all practices and competitions unless excused by the coach.

• The athlete completes all Physical Fitness Testing as required by the P.E. Department.

• The petition has been validated by the Athletic Director.

    Eligibility — To be eligible to participate, an athlete must meet the following requirements:

• An athlete must be enrolled in six classes per day.

• An athlete is ineligible when receiving either two (2) “D’s” or one (1) “F” for his/her comprehensive grade in any class. If this occurs he/she will be ineligible until the grade is brought up. Weekly academic checks are done by the athletic director. When an athlete is academically ineligible his/her parent(s)/guardian(s) will be notified by the coach or athletic director.

• Students on disciplinary and/or academic probation may have their privilege to participate limited.

    Athlete School Attendance — When an athlete is absent from school  on the day of an event, he/she may not participate in any extracurricular sport activity for the day (see attendance policy). This is primarily a safety factor for the athlete, as well as for the school. A pre-approved absence by the school is an exception. It remains the responsibility of the individual athlete to present written evidence of mitigating circumstances which justify any exception as to the athlete’s attendance. (Any student participating in an event when not in all classes that day may be prohibited from participating in the next scheduled event.) Athletes are to be in regular school attendance the day following a contest if school is in regular session.

Any athlete who is considered to have an unexcused absence from any class the day of an event will be ineligible to compete. (Any athlete participating in an event when ineligible will be prohibited from participating in the next scheduled event.) Parent(s)/guardian(s) will be notified when this has occurred.

    Equipment — All athletes are responsible for the proper care and security of all equipment issued to them. Please note that:

• School furnished equipment is to be worn only for contests.

• All equipment not returned will result in a financial penalty.

    Number of Sports — Athletes may participate in only one IHSA sport per season unless permission is granted from the Athletic Director.

    Dropping or Transferring Sports — Quitting is an intolerable habit to acquire. On occasion, however, an athlete may find it necessary to drop a sport for a good reason. If this is the case, the following procedure must be followed:

•   Consult your coach.

•   Report your situation to the Athletic Director.

•   Return all Uni High equipment issued to you.

•   Return to Physical Education class the day after your last practice/event.

    Use of Tobacco, Alcohol or Drugs — A large part of being an athlete is taking proper care of the body. It has been proven through extensive research that the use of tobacco, alcohol and drugs is harmful to the human body. Therefore, Uni High will not permit the use of any of these substances.

    Travel — All athletes must travel to and from athletic contests and practices in transportation provided by the Athletic Department unless previous arrangements are made by the parent(s)/guardian(s) for exceptional situations. Please note that:

• All athletes will remain with their team and under the supervision of the coach.

• All regular school bus rules will be followed.

• All athletes will dress appropriately and in good taste on contest days.

    Missing Practices — An athlete must consult his/her coach before missing practice.

    Individual Team Rules — All teams will have a list of team rules and regulations. These rules and regulations have been approved by the administration and are to be followed in the same way as departmental rules and regulations.
