2.2 REGISTRATION

Prior to the start of the school year, students receive a registration packet. New students must bring one legal form of identification to the main office; for example, a birth certificate, passport, or hospital record of birth with foot and hand prints. (NOTE: A copy will be made for school records and the original returned immediately.) Initial enrollment involves filling out consent forms that stay on file until after graduation. During orientation, new students receive their Uni High identification card. Returning students can mail their registration information and fees to the school.
n Required Forms — The following forms must be completed at registration:

n Physical Examinations — All new students and freshmen must have a current physical examination and turn in an exam form provided by the school when they register. If a student does not present proof of health examination and immunizations by November 1 of the year in which such proof is mandated, the school must suspend the student from school.
Students must include a complete immunization record. To be in compliance with state laws, the following vaccinations are required:
• Polio — Three or more doses administered at the appropriate age, with the last dose being a booster and having been received on or after the 4th birthday.
• DPT — Three or more doses administered at the appropriate age, with the last dose being a booster and having been received on or after the 4th birthday.
• MMR — Must have received two doses of MMR vaccine, with the first dose administered no earlier than 12 months of age.
• Hepatitis B — All students entering 5th grade 1997-98 or after are required to have a series of 3 shots. Any students in higher grades are not required to have this series of shots.
NOTE: Student athletes must turn in each year an IHSA physical examination form and medical emergency information form when registering in the Athletic Office.
NOTE: International Exchange and Visiting Students must comply with the above physical examination requirements and with IHSA eligibility rules in order to participate in any IHSA sanctioned events.
n Student IDs — Each new student will have an identification card issued with her/his picture and a nine-digit identification number on it during the first few weeks of school. In the junior year, students will be reissued an ID with a current picture. With their school ID card, Uni students have access to all university libraries, computer labs, and the Illini Union, and are expected to abide by university regulations in these facilities. If a student loses his/her I.D., the students should see the receptionist in the Main Office for an I.D. form to take to the Campus I.D. Center located on the second floor of the Illini Union Bookstore. The cost for the lost I.D. is $20.00.
